Manager, Case Management (NYS RN Required)
The Manager of Case Management supervises team of RN Case Managers working both in the Inpatient and Outpatient setting. The role involves using an interdisciplinary collaborative approach to fulfill responsibilities that include utilization, concurrent review, discharge planning, coordination of care, patient education, working to improve patient care at MSK. Implement initiatives that promote an environment assuring seamless ongoing access to care.
Advise on complex case management/discharge planning issues assisting staff with problem identification and resolutions.
Develop and foster relationships with multiple counterparts and key stakeholders across MSK.
Provide ongoing supervision, feedback and evaluation of the Case Managers and ancillary staff.
Respond to the changing needs of the institution and the patient population and contribute to the development of new programs.
Conduct hospital-wide educational in-services pertaining to the role of the Case Manager and changes in healthcare reimbursement.
Current NYS Registered Nurse License/Registration
Bachelors of Nursing (BSN) degree required; Master’s degree in nursing is highly preferred
4-7 years of clinical nursing experience
Previous case management, home care, managed care, or UM/CM experience required
Certified Case Manager (CCM) or Accredited Case Manager (ACM) status preferred,
Excellent interpersonal, problem solving and communication skills essential
Strong decision-making and negotiation skills
Knowledge of state and federal regulations that govern health insurance coverage
Knowledge of Microsoft: Outlook, Word, Excel, Power Point, and Internet Explore
