The goal of the Vice-Chair for Quality and Safety in the Department of Radiology at Memorial Sloan Kettering Cancer Center (MSK) is to establish and maintain a robust quality and safety program, and facilitate resultant process improvements, driven by data.
Primary responsibilities include:
Represent the Radiology Department on Hospital Leadership committees pertaining to quality, safety, and performance improvement.
Develop, grow, and sustain a program within the Department of Radiology centered around patient safety, process improvement (PI), professional outcome assessment, and patient satisfaction.
Instill a culture of safety and quality throughout the Department.
Oversee Radiology Quality Assurance activities across all sites, responding to critical or escalating QA issues with change implementation plans if needed
Establish and manage a Radiology quality and safety management team, composed of physicians, nurses, technologists, managers and others, that meets regularly to discuss, evaluate and manage all components of Radiology quality and safety.
Monitor, participate in, and facilitate clinical operations safety programs, including radiation safety, MRI safety, universal protocol, and Joint Commission requirements
Establish compliance with National Patient Safety Goals (NPSG) through formulation, implementation, and regular review and upgrading of departmental policies in all areas of patient safety (including time outs, patient identification, site and side marking, patient falls, medication reconciliation, and any new goals introduced).
Establish a program of walk rounds for real-time assessment of safety and quality issues.
Manage reporting of, and responses to, adverse events in Radiology, and reduce the risks of adverse events by ongoing analysis of reported and potential adverse events and near misses.
Establish key performance indicators (KPI) relating to patient safety, access to services, and customer satisfaction - including a special focus on safety issues such as patient identification, patient falls, contrast reactions and extravasation, procedural complications, radiation exposure, and diagnostic errors.
Develop mechanisms for collecting underlying data needed to measure KPIs, tools to efficiently generate KPIs in an actionable timeframe, including a quality scorecard that posts relevant data for each operational group within the Department.
Establish and support PI projects within all Radiology Services
Monitor and manage a peer-review process for faculty members, to address:
Joint Commission-required initial Focused Professional Performance Evaluation (FPPE) and periodic Ongoing Professional Performance Evaluations (OPPE) of each faculty.
American College of Radiology (ACR) requirements for peer review of radiologists, to achieve and maintain ACR accreditation of our radiology facilities.
Assist in preparation for, and participate in, site visits by regulatory and accreditation bodies.
Organize and lead relevant educational activities in quality, patient safety, and PI for faculty and staff.
